Anticoagulation Safety Program: A Multipronged Strategy

Presenter:	Bob Lobo, PharmD, FCCP, BCPS, graduated from the University of Nebraska College of Pharmacy and completed an ASHP-accredited pharmacy residency training program at the University of Tennessee College of Pharmacy in 1989. Following his residency, Dr. Lobo accepted a position as a pharmacist at Methodist University Hospital in Memphis, TN. In this position, he established clinical pharmacy services to adult inpatient psychiatric, geriatric, neurologic, and internal medicine patients and was a part-time faculty member of the University of Tennessee College of Pharmacy. He was also an active preceptor in the hospital’s pharmacy residency program. Dr. Lobo transitioned from clinical practice to a clinical leader role in 2006. In 2008, he moved to Nashville, where he currently serves as Director, Clinical Programs in the Department of Pharmaceutical Services at Vanderbilt. In this role, Dr. Lobo is engaged in clinical program development, medication use policy, clinical decision support and oversight of educational programs for students, residents and staff.
Continuing Education:	This seminar will provide one hour of continuing education credit to pharmacists who attend the session and complete the online activities (activity evaluations and/or post-tests) required to claim credit. The Tennessee Pharmacists Consortium for Education is accredited by the Accreditation Council for Pharmacy Education, as a provider of continuing pharmacy education.
Learning Objectives:	Dr. Lobo plans for participants to be able to identify key components of a multipronged anticoagulation safety program, recognize strengths and weaknesses of various anticoagulation safety strategies, and describe the pharmacists’ role in improving anticoagulant safety.
